Housing Supports and Services
Housing Supports and Services (Housing Outreach Team and Palm 2 PSH) provides permanent supportive housing to approximately 240 households throughout Multnomah County including adult-only households, couple households and families with children. These programs provide home and community based services, including housing search and placement services, housing retention services, mental health services — including case management, skills training, counseling, medication services and crisis intervention services. Most of these projects are funded via HUD and serve individuals and families experiencing chronic homelessness, where the head of household has a mental health disability and is Medicaid eligible. These participants are referred through the Coordinated Access system with Multnomah County,
Two other PSH projects receive referrals directly through internal Cascadia programs and serve individuals and families with children who have a mental health disability, are Medicaid eligible, and who are experiencing homelessness or housing instability. Short Term Rental Assistance (STRA) is also administered via the Housing Outreach Team for Cascadia-enrolled clients to provide financial assistance for homelessness prevention and rapid rehousing.
