Program staff will still provide phone support to ensure clients are able to get medications, medical treatment and basic needs are being met. Face-to-face services will be reserved for the highest acuity and those who cannot be reached by phone.  Staff will not go into home, and will maintain a six-foot distance. Referrals will be reviewed and evaluated to determine the safest way to screen and assess each new potential client.

Housing Outreach Team (HOT)

The Housing Outreach Team (HOT) operates and administers a variety of permanent supportive housing in Multnomah County to more than 225+ households including single adults, couples and families with children, including individuals without insurance or without income.  These programs provide a rental subsidy and supportive services to participants who have experienced homelessness and have a mental health disability.  Services are mobile and provided in the participants’  homes or in the community with the intent of meeting participants where they are. HOT services are varied and include housing placement, housing retention, and mental health services including outreach and engagement, assisting in housing search and application processes, moving in, obtaining furniture, tenancy skills, advocacy with landlords, counseling, case management, support in accessing primary care, and addressing health needs.
